There is a  requirement of ONE  (1) Meningococcal vaccine on of after age 11 for 6th Grade Entry

and TWO (2) Doses of Meningococcal vaccine for 12th Grade Entry.  A booster dose should be given if at least 4 years have elapsed between the first dose.
